DESCRIPTION:
Topical steroids with anti-infectives are combination creams that contain both a steroid and an anti-infective agent and are used to treat infections of the skin or scalp that are red, itchy, or have minor swelling. Steroids (also known as corticosteroids) are very effective at reducing inflammation (redness and swelling), suppressing the immune system, and constricting (narrowing) blood vessels in the skin. They are available in various potencies (strengths). Anti-infective agents kill or inhibit the spread of infectious organisms. They include antibacterials (suppress bacterial growth), antifungal (kill or suppress fungi) agents. The choice of topical steroid with anti-infective depends on the suspected or confirmed infective organism (ie, whether it is a bacteria or fungus) and the potency of the steroid required. Ketoconazole contains the broad-spectrum synthetic antifungal agent, Ketoconazole 2%. Ketoconazole is an imidazole antifungal agent Beclomethasone Dipropionate is a potent glucocorticoid steroid. It is a prodrug of the free form, Beclomethasone. Neomycin is a topical antibiotic for the treatment or destroys the fungi and bacteria which cause skin infections.

COMPOSITION:

MECHANISM OF ACTION:
Azoles such as ketoconazole have additional anti-inflammatory action which can be useful in reducing the symptoms. Topical steroids control the biosynthesis of potent mediators of inflammation such as prostaglandins and leukotrienes by inhibiting the release of their common precursor, arachidonic acid. Topical corticosteroids have anti-inflammatory, antipruritic, and vasoconstrictive properties. Corticosteroids are thought to act by the induction of phospholipase A2 inhibitory proteins, collectively called lipocortins. It is postulated that these proteins control the biosynthesis of potent mediators of inflammation such as prostaglandins and leukotrienes by inhibiting the release of their common precursor arachidonic acid. Arachidonic acid is released from membrane phospholipids by phospholipase A2. Topical antifungals are an important adjuvant in the treatment of dermatophytosis.

Ketoconazole kills fungi and yeasts by interfering with their cell membranes. It works by impairing the synthesis of ergosterol, which is an essential component of fungal cell membranes. The disruption in the production of ergosterol causes holes to appear in the fungal cell membrane. The cell membrane of fungi is vital for their survival. They keep unwanted substances from entering the cells and stop the contents of the cells from leaking out. As ketoconazole causes holes to appear in the cell membranes, essential constituents of the fungal cells leak out. This kills the fungi and clears up the infection. Beclomethasone, a corticosteroid (cortisone-like medicine or steroid), is used to help relieve redness, swelling, itching, and another discomfort of fungus infections. Neomycin mediates its bactericidal action by inhibiting bacterial protein synthesis, thereby suppressing the growth and survival of susceptible bacteria.

INDICATIONS:
Athletes Foot, Jock Itch, Ring Worm, Seborrhea (Dry, Flaking skin), Intertrigo, Infection between Toes & Fingers & Eczema.

INSTRUCTIONS FOR USE:
The affected area should be cleaned and dried thoroughly before the application of Cream. The cream should be applied to the affected skin and surrounding area in a thin layer and rubbed in lightly. In the case of intertriginous infections (interdigital, intergluteal, inguinal) the application may be covered with a gauze strip, especially at night.

CONTRA-INDICATIONS:
Known hypersensitivity to corticosteroids.
Infections of the skin.
Age under 2 years.

SIDE EFFECTS:
Existing infections may be exacerbated. Prolonged use can induce atrophy of the skin, particularly on the face and in skin folds. This is characterized by thinning of the dermis, depigmentation, dilatation of superficial blood vessels and formation of striae. Infants and young children are particularly susceptible to the local and systemic effects of topical corticosteroids. Prolonged use can result in hypercorticism and suppression of the hypothalamic-pituitary-adrenal axis.

DOSAGE:
As directed by the physician.

PRECAUTIONS:
Prolonged therapy or treatment of large areas of skin should be avoided as adrenal suppression can occur. It should not be used on the face for more than 7 consecutive days. If secondary infection occurs during treatment, it should be withdrawn and an appropriate antimicrobial given.

USE IN PREGNANCY:
It should not be used in large amounts or for more than 2 weeks during pregnancy.

STORAGE:
Keep in a cool place, Protect from light, DO NOT FREEZE.
Keep the tube tightly closed after use.
